Advance Care Planning Makes CMS’s Measures List
A draft list of measures by the Centers for Medicare and Medicaid Services (CMS) for Medicare programs like the Quality Reporting Program and the Value Based Purchasing Program includes one that could impact skilled nursing facilities (SNFs): advance care planning.
The measure would track the percentage of adult patients who have an advance care planning document or proof of an advance care planning discussion resulting in a documented decision in the electronic health record by discharge.
The advance care planning measure is one of 25 included in the 2025 list of measures under consideration.
“Twenty-nine percent (7 of 24) of the measures address the Chronic Conditions and Related Acute Events Meaningful Measure Priority, while 25 percent (6 of 24) of the measures address the Safety Meaningful Measure Priority, reflecting the importance in improving healthcare quality and patient well-being,” CMS said.
