The Centers for Medicare and Medicaid Services (CMS) has a new form for beneficiaries to submit complaints to the agency about Medicare Advantage (MA) plans.

CMS will take complaints via the form and then send them to the Health Plan Management System Tracking Module (CTM). From there, CMS said it will assess the issues at hand and process the complaint information. MA plans will not receive the original provider complaint form.
